Health Measuring Tools
Compliance with the common rules requires the use of validated tools shared by the international scientific community. These tools allow to assess the needs / problems of the patient, to share the classification, to improve the measure and codify the language.
Rating scales and welfare indexes are measuring instruments, which identify objectively and accurately the needs of a person. These tools can be shared, using a common international language, can measure several observable phenomena and facilitate the formulation of predictions and explanations.

This app is an experimental project aimed to provide the health operators with a simple free tool for record, read and analyze data from assessment tools. Our commitment is to add and enhance it with more scales and indexes with the help of the users suggestions and contributions.
In the next paragraphs we will describe, step by step, the functions included in the application.
QUICK REFERENCE GUIDE
This is the start screen. On the bottom bar there are two buttons:
Scales: for browsing the available scales and detailed information (fig. 2)
Patients: for browsing the list of patients (fig.4)
Info Scale Screen
It displays a list of the available scales installed in the application. The number and types of the scales can be different depending on the language you use as default on your device. We used only public translations and in some cases we couldn't find them in all the languages.
The blue right arrow allows to display the detailed information and bibliography for the scale and the scores used (fig. 3).
To ease the reading of the scores the app uses colors that mimic the risk level or the autonomy for the assessments. Green color is used for low level or no risk or complete autonomy; the color changes to yellow, red and bright red as the risk increases or the autonomy decreases (fig. 3)
Patients List Screen
It displays the whole list of patients you create in HMT. There is a default patient ,with some assessments, for learning/introduction purpose (you cannot edit/delete this record).
Use the detail button on the right for to display the list of all assessments done for any patient (fig. 5).
Simply tap on the name to access the patient record card.
Use the "New" bottom button to create a new patient.
Wipe from right to left on a patient to display the delete button for the record.
As mentioned before, in this screen you see the list of the assessments for the patient. The colored circles give a fast interpretation for the scores on each assess. You can use the right blue button present on each assessment to display the full detail of it. You can only modify assessments that have been done on the current date (fig. 6).

It displays the categories and checks you did in the assessment. As usual, you have a right blue button to expand the information for any item.

The patient data: name, surname, age, etc.
There is also a special button to set/change the
BMI (Body Mass Index) (fig. 8), the BMI will be linked with the assessments done after the change.
Use "New Asses" button to select an index and set the data of a new assessment (fig. 9)
Use "History" button to display graphically the history of the assessments for the patient (Fig. 12)
Useful to compute the body mass index. The calculated value is also displayed on the graphical history (Fig. 12). On the patient's card is shown the last index.
Available scales list
On this screen are shown the available scales/indexes you can use for the assessments. A tag will be set when you finish to make an assess indicating that you already used it (Fig. 11). The date of the assessment is always the date of the device.
You can only modify assessments done on the current date.
Assessment input screen
Each scale has a list of items/questions grouped by category. The score is computed as you set the tags and the total displayed on the last line of the list.
Used scales example
As said before, the used scales have a tag set. The tag is only valid for the day you made the assessment.
History or temporal graph
The scores calculated for the scales are displayed on a temporal graph with the BMI as a colored bar on the top of the graph. If there are more than one scale in use for the patient, you can select the scale to display with the top right button.
The assessment for the graph are displayed in a list on the bottom of the graph. As usual you can expand any item on that list tapping on it.
